Medications ordered for school administration

 1 need a signed permission form from parent/guardian
 2 need a form with physicians orders and signature
 3 will have medications brought to school by parent/guardian
 4 medications will be given to nurse or front office staff


 Over The Counter Medications

 1 need to have a permission form signed by parent /guardian
 2 the form needs to have the students full name and parents signature
 3 the form needs to include the last dose the parent /guardian gave
 4 the form needs to have time frame to dispense medication i.e. 10 days
 5 medication will be brought to school by parent/guardian

 Any medications left at school must be wasted after time to dispense is up

 Students ordered special screening  i.e.blood pressure, heart rate, blood sugars

 1 need order from the physician
 2 need permission form signed by parent/guardian
